In view of the current haze situation, the school is updating our student Health and Support Records to ensure that we are better prepared to provide appropriate assistance, precautions, and support where needed.
Parents/Guardians are requested to complete the form if your child has:
• Any health condition that may be affected or aggravated by the haze, such as asthma, allergies, respiratory conditions, or other relevant health concerns; and/or
• Any other health, medical, mobility, learning, emotional, dietary, or safety-related matter that you feel the school should be aware of in order to support your child better/necessarily.
